Choosing the correct imaging modality for kidney stones involves many factors including the clinical setting, patient body habitus, cost, and tolerance of ionizing radiation. Multiple imaging modalities are available, but widespread clinical use is currently limited to CT, ultrasonography, and kidney ureter bladder (KUB) plain film radiography.

With US, larger stones (>5–7 mm) within the kidney, i.e., in the calyces, the pelvis and the pyeloureteric junction, can be differentiated, especially in the cases with accompanying hydronephrosis (Figure 18 and Figure 19) [26,29]. Hyperechoic stones are seen with accompanying posterior shadowing.

Kidney Stone. This patient had pain in the right lumbar region. Sonography of the kidneys reveals an markedly echogenic lesion in the lower calyx of right kidney. An acoustic shadow is seen posterior to the lesion. These ultrasound images are diagnostic of a renal calculus (measuring 15 x 10 mm. in size) in the lower calyx.
