2024年11月5日 · Contrast media extravasation (CMEV) refers to the leakage of intravenously administered contrast media from the normal intravascular compartment into surrounding soft tissues; it is a well-known complication of contrast-enhanced CT. It can also occur in MRI, but the complications are rare given the low volumes that are used.

Below is a summary of the recommendations related to contrast extravasation and the associated strength of evidence for those recommendations using the ACR Appropriateness Criteria® Methodology.

If the contrast injection leaks out from the vein to the tissues under your skin, this is called extravasation. It can cause a stinging feeling, but this usually wears off after about 30 minutes. You might also have some swelling in your arm or hand.

Extravasation of contrast is a possible complication of imaging studies performed with contrasts. Most extravasations cause minimal swelling or erythema, however, skin necrosis, ulceration and compartment syndrome may occur with extravasation of large volumes of contrast.

Extravasation occurring with dynamic bolus CT may involve large volumes of contrast media. Although most patients complain of initial swelling or tightness, and/or stinging or burning pain at the site of extravasation, some experience little or no discomfort.

We propose a set of guidelines for the initial management of contrast media extravasation injuries for a timely intervention. For large‐volume extravasation cases, immediate management with multiple slit incisions is safe and effective in reducing the swelling quickly, preventing patient discomfort and decreasing skin and soft tissue problems.

Areas covered: In this paper, we review the literature pertaining to the pathophysiology, diagnosis, risk factors and treatments of contrast media extravasation. A suggested protocol and guidelines are recommended based upon the available literature. Key points: • Risk of extravasation is dependent on scanning technique and patient risk factors.
