Researchers are encouraged to use this when creating their informed consent forms to best suit the design of their study. Use of alternative wording or format is allowed. The informed consent form consists of two parts: the information sheet and the consent section. Do not be concerned with the length of this guide.

The researcher signature section below is not required, but is recommended for research studies involving an in-person consent procedure, especially when consent may be obtained by multiple members of the research team.
