Masseter muscle hypertrophy is a rare condition of idiopathic cause. It clinically presents as an enlargement of one or both masseter muscles. Most patients complain of facial asymmetry; however, symptoms such as trismus, protrusion, and bruxism may also occur.

Generalised masticatory muscle hypertrophy may affect the temporalis muscle, masseters and medial pterygoids in a variety of combinations. Masseteric hypertrophy may present as either unilateral or bilateral painless swelling of unknown origin in the region of angle of mandible. It is a relatively rare condition and presents a diagnostic dilemma.

Data from clinical and radiographic exami-nation led to the diagnosis of bilateral idiopathic masseter muscle hypertrophy with retrognathic mandible. Surgery was offered for cosmetic reasons. Bilateral reduction of bone at the mandibular angle and an anterior repositioning of mandible by 8 mm were planned. Figure 1. Figure 2. Figure 3. Figure 4.
